Details needed at the meeting

I've no idea how many will turn up, but we'll need to collect contact
details of those who are interested in staying involved. Not all will be
online.
For the sake of confidentiality I think we should give out individual forms.
For accuracy and efficiency it could be done by a form on a laptop but
possibly a bit late for that, also wd need to observe the Data Protection Act.
The form, physical or electronic, should ask the following (all questions
optional):
What name/nickname do you wish to be known as?
Do you represent or are you associated with any relevant organisations,
please list
Can you help with
. lobbying
. publicity
. etc, etc
. other (please specify)
Please give us at least one way to contact you. In order of preference:
. e-mail address
. fax number
. name and postal address
. telephone no
[small print, a promise that we won't pass on the dets and keep in
confidence. We'll contact with relevant info from time to time, and will
stop if asked]
Anything else?

No, I'll let you do that if they're different for each. I could run off a
few for those who want to get notified of things and aren't offering any
particular specialism.
Up against a deadline for work, sod's law timing.
BTW are we asking for donations from people, wd be an idea to help cover
costs.
